Neil, those are not wheels. Those are the two cooling fans for the A/C equipment. The two wheels, which can be seen at the front end of the coach are well and truly on the track. The rear wheels are not in the frame of the picture. However, you can see just a tiny bit of the rear wheel bogie.

Braganza ghat (and Dudhsagar waterfalls) are located between Kulem and Castle Rock. One bifurcates from the KR at Madgaon to reach Kulem.
